Fertility Tests for Couples: Process, Timeline, and Cost in India

When a couple is trying to conceive and pregnancy does not occur after 12 months of regular unprotected intercourse (or after 6 months if the woman is over 35), doctors recommend a fertility evaluation. Fertility testing helps identify whether the difficulty lies with egg reserve, fallopian tubes, the uterus, sperm health, or hormonal factors.

Importantly, infertility is not only a female issue. Medical data shows that male factors contribute to infertility in nearly 40–50% of couples, which is why both partners should undergo evaluation together.

Basic Fertility Tests for Women

Female fertility testing mainly focuses on three important areas: ovarian reserve (egg count), fallopian tube patency, and the health of the uterus.

  1. Ovarian Reserve Testing (Egg Count)

Ovarian reserve tests estimate how many eggs remain in the ovaries. These tests help doctors understand how a woman may respond to fertility treatments.

Common tests include:

  • AMH (Anti-Müllerian Hormone) blood test
  • FSH, LH, and Estradiol hormone levels
  • Antral Follicle Count (AFC) measured by ultrasound

AMH has become one of the most widely used markers of ovarian reserve and is helpful in planning fertility treatment.

  1. Transvaginal Ultrasound Scan

A transvaginal ultrasound is an essential fertility test that evaluates the reproductive organs in detail. It helps doctors assess:

  • Uterine shape and lining (endometrium)

  • Ovarian cysts
  • Antral follicle count (egg reserve)
  • Fluid-filled fallopian tubes (hydrosalpinx)
  • Adenomyosis or other pelvic abnormalities

  1. Fallopian Tube Test

For natural conception to occur, the fallopian tubes must be open so that the egg and sperm can meet. The most common test used to check this is Hysterosalpingography (HSG).

In this procedure, a dye is injected through the cervix and X-ray images are taken to confirm whether the tubes are open.

  1. Advanced Diagnostic Tests

In certain cases, doctors may recommend hysteroscopy or laparoscopy. These procedures allow direct visualization of the uterus and pelvis and can diagnose conditions such as endometriosis, adhesions, or uterine abnormalities.

  1. Pre-Pregnancy Health Screening

Before planning pregnancy, doctors may also recommend basic blood tests to ensure the body is medically ready for conception.

  • Thyroid function (TSH)
  • Blood sugar
  • Prolactin level
  • Thalassemia screening
  • Rubella immunity

Fertility Tests for Men

Male fertility evaluation is usually simpler and begins with a semen analysis, which assesses sperm health.

The test evaluates:

  • Sperm count (number of sperm)
  • Motility (how well sperm move)
  • Morphology (shape of sperm)
  • Semen volume and overall quality

The sample is collected after 2–5 days of abstinence for the most accurate results.

If abnormalities are found, doctors may recommend additional tests such as:

How Long Does Fertility Testing Take?

Most basic fertility tests can be completed within one menstrual cycle. Blood tests and semen analysis can be done immediately, while ultrasound and tubal tests are scheduled according to the menstrual cycle.

For most couples, the basic fertility evaluation costs between ₹15,000 and ₹25,000 depending on the number of tests required. Advanced procedures like hysteroscopy or laparoscopy may increase the total cost.
